Any option from switching form Xarelto 15mg q12h to IV heparin?

Good morning

Any option from switching form Xarelto 15mg q12h to IV heparin?
Usually it’s the next scheduled dose but in this case we started loading dose, so can it be started in 12 hours still?
T ½= 8-9 hours (crcl>50)
Pt is 80 yo, 63 kg, cr=0.6

Also how long after last dose of Xarelto to do thrombolectomy?
