NY, DC, SC: Program to promote diabetes self-management for underserved population
IPRO, Delmarva, and the Carolinas Center for Medical Excellence of Columbia have partnered to form the Atlantic Quality Innovation Network (AQIN), under contract with the Centers for Medicare & Medicaid Services (CMS). AQIN is working to promote a diabetes self-management program for underserved Medicare beneficiaries living with diabetes. This program, called “Everyone with Diabetes Counts”, focuses on increasing patients’ knowledge of diabetes management. It also works to improve healthcare provider use of important diabetes management tests through community and partnership outreach.
