Has anyone done a patient experience survey for anticoagulation clinics?

I am a pharmacist with an anticoagulation clinic in Minnesota and we are looking to do our first patient experience survey. Wondering if anyone has past experience with this they'd share with us. Any questions/questionnaires that worked well that you'd be willing to share?
